From referral to classroom interventions, the Dikeos Behavior Inventory documents and directs the process to assure that behavioral issues are addressed.
This four page form and checklist diagnoses misbehavior patterns into up to four categories, Impulsive, Rebellious, Discouraged, and Denial.
Page one starts the behavior problem-solving process by asking the teacher to identify the problem behavior, desired behavior, when the problem behavior does not happen, and if those conditions can be replicated.
There is a place for parents to be asked for strengths and interests of their child and other factors that the child may be dealing with that should be considered.
The inside is a checklist and scoring sheet to diagnose behavioral patterns. The back will structure the multi-disciplinary team meeting.
The diagnosis is correlated with suggested Empowered Discipline™ strategies. This is followed by a space for planning who will implement the strategy selected and when it will go into effect and be reevaluated.
