Statins
Statins are a class of drugs used to lower blood cholesterol.
These include: Lovastatin (Mevacor, Altocor), Fluvastatin (Lescol), Atorvastatin (Lipitor), Pravastatin (Pravachol) and Simvastatin (Zocor).
All statins act on an enzyme in the liver that controls production of cholesterol. Statins have many benefits, and in some people, they reduce the risk of heart attack and stroke. Like all medications, statins have potential side effects. Fortunately, statins are well tolerated by most people.
The most common side effects of statins are: Nausea, Diarrhea, Constipation, and Muscle aching.
Occasionally, statin drugs cause elevated levels of liver enzymes. If the increase is mild, you typically can continue taking the drug. If the increase is severe, you may need to discontinue it. Usually, liver abnormalities due to statins improve after you stop taking the drugs. Certain other cholesterol-lowering drugs, such as gemfibrozil (Lopid) and niacin, increase the risk of liver toxicity when taken with statins. Because liver problems may develop without causing symptoms, people who take statins should have their liver function tested periodically.
Statins also may cause muscle pain and tenderness due to statin myopathy. In severe cases, muscle cells break down (rhabdomyolysis) and release the protein myoglobin into the blood. Excretion of myoglobin into the urine can impair kidney function and lead to kidney failure. Certain drugs when taken with statins increase the risk of rhabdomyolysis.
These include: Gemfibrozil (Lopid), Erythromycin (Erythrocin), Clarithromycin (Biaxin), Antifungals, such as ketoconazole and itraconazole, Nefazodone (Serzone), Cyclosporine (Sandimmune, Neoral) and Niacin.
If you take statins and have new muscle aching or tenderness, consult with your doctor. Also, avoid taking statins with grapefruit juice, which alters your body's metabolism of these drugs.
Because the body makes most of its cholesterol at night, doctors generally recommend that people take statins late in the day.

Olymethoxylated Flavonoids - Preclinical studies have shown that the specific citrus polymethoxylated flavonoids found in Cholestin, tangeretin and nobiletin inhibited the secretion of apo B, an LDL-C precursor, from HepG2 cells (Kurowska, 2004a; 2002). A separate study showed that tangeretin inhibits apoB production by suppressing diacylglycerol actyltransferase, the final enzyme in the pathway of triglyceride synthesis, where the latter plays an important role in apoB formation (Staack, 2005). In liver cells, tangeretin rapidly reduced apoB secretion (82% reduction); it also decreased intracellular synthesis of cholesteryl esters (45% reduction), free cholesterol (64% reduction), and triacylglycerols (37% reduction). This suppression of TAG synthesis and mass were associated with decreased diacylglycerol (DAG) aceyltransferase and microsomal triglyceride transfer protein (MTP) activities (Kurowska, 2004b).
In addition to its ability to reduce plasma cholesterol concentrations, nobiletin was shown in vitro to inhibit macrophage foam cell formation (Whitman, 2005).
Tocotrienols - A number of clinical studies using tocotrienols derived from palm fruit or rice bran have been performed. Subjects receiving tocotrienol supplementation for 4-8 weeks experienced significant drops in total cholesterol of 15-20%, with the major reduction being in LDL-C, whereas HDL-C remained essentially unchanged. In a trial with pure gamma tocotrienol, similar reductions were also observed (Theriault, 1999).
Geraniol - Geraniol has been shown to exhibit potent HMG-CoA reductase activity in vitro, more powerful than the hypolipidemic drug mevacor.
These results indicate that geraniol primarily suppresses HMG-CoA reductase synthesis at a post-transcriptional level by attenuating HMG-CoA reductase mRNA translational efficiency. (Peffley, 2003).
